Job Summary

The Registered Nurse (RN) in our Older Adult Specialized Seniors Clinic provides high-quality patient care by assessing, planning, implementing, and evaluating individualized care plans. This role requires collaboration with the interprofessional care team to ensure comprehensive care delivery and patient-centered services.

  • Key Responsibilities:
  • Assess patients referred to the clinic, collecting information from various sources, and making nursing diagnoses.
  • Develop and implement individualized care plans, working closely with patients, families, and caregivers.
  • Deliver patient education on geriatric health issues, illness management, and prevention strategies.
  • Collaborate with the interprofessional care team to coordinate care and make referrals as needed.
  • Document patient assessments, care plans, and outcomes according to practice standards and Fraser Health procedures.

Requirements:

  • Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years of recent related acute care experience working with a geriatric population, or an equivalent combination of education, training, and experience.
  • Ability to communicate effectively, promote person-centered care, and integrate data from multiple sources to problem-solve effectively.
