Healthcare Leader for Integrated Care Programs

Spectrum Health Care, a leading provider of innovative home health care and specialized services, is seeking an experienced Registered Nurse to serve as Program Manager for our Palliative Care program. With over 40 years of experience delivering high-quality, integrated care, we are committed to driving positive health system change.
About the RoleThis is a dynamic leadership opportunity that requires a seasoned healthcare professional with expertise in program development, team management, and collaborative practice. As Program Manager, you will lead a multidisciplinary team to deliver seamless, holistic care to patients, families, and caregivers in Mississauga. Your primary responsibilities will include:
- Developing and implementing clinical workflows to support innovative models of care
- Recruiting, onboarding, and mentoring team members to ensure they have the necessary skills and training
- Leading and mentoring the team to consistently meet high care quality standards
- Collaborating with operational leadership to implement exemplary practices
- Addressing quality, safety, and risk issues, and developing improvement plans as needed
In this role, you will work closely with external stakeholders to design and implement innovative programs, ensuring seamless care transitions across the broader health system.
RequirementsTo be successful in this role, you will require:
- A current CNO certificate as Registered Nurse, degree preferred
- Minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager, preferably in a community setting
- Successful completion of one or a combination of the required certifications, including Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; Learning Essentials Approaches to Palliative and End of Life Care (LEAP); Fundamentals of Palliative Care; or The Comprehensive Advanced Palliative Care Education (CAPCE)
- Experience managing in a union environment is an asset
- Solid knowledge of the principles, practices, and methods related to leading multidisciplinary teams
- Experience with program development, implementation, and evaluation
- A valid G driver's license and reliable vehicle
- A proven track record of effectively handling complaints and resolving conflicts
- Ability to work independently and effectively lead a team
- Ability to understand the different needs of various partners and ability to drive collaborative solutions
We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, plus benefits and opportunities for growth and development.
