Job Title: Palliative Care Team Lead

About the Role:

Spectrum Health Care is seeking a highly skilled and compassionate Palliative Care Team Lead to join our team. As a key member of our Integrated Specialty Programs, you will have the opportunity to cultivate and manage high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals.

In this role, you will be responsible for ensuring seamless program operations, leveraging data-driven insights to optimize service delivery, enhance quality, support team members to work to full scope of practice including indirect care coordination activities, and achieve key outcomes.

The ideal candidate will have a strong background in palliative care nursing, with a minimum of 5 years of experience in a community setting and 2+ years in a management role. A valid G driver's license and reliable vehicle are also required.

Key Responsibilities:
  • Lead an integrated palliative care team within a designated neighbourhood, managing day-to-day coordination of services ensuring continuity of care, seamless processes and optimal patient and employee experience.
  • Cultivate and maintain effective relationships with internal and external stakeholders, including patients, caregivers, and other healthcare professionals.
  • Develop and implement comprehensive care plans that respect patient wishes and priorities, with a focus on providing high-quality care and promoting patient-centered decision-making.
  • Oversee and monitor indirect care coordination activities provided by regulated and unregulated health professionals on your care team, ensuring that all team members are working collaboratively to meet patient needs.
  • Foster a culture of excellence and continuous improvement, encouraging team members to share ideas and best practices to improve patient outcomes.
  • Collaborate with other healthcare professionals to address quality, safety, and risk issues, developing improvement plans as needed and participating in Quality-of-care Reviews as required.
Requirements:
  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
  • Minimum of 5 years nursing experience in a community setting and 2+ years in a management role.
  • Valid G driver's license and reliable vehicle.
  • Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or equivalent experience.
What We Offer:

Spectrum Health Care offers a competitive salary range of $85,000 - $105,000 per year, depending on experience, plus benefits and opportunities for professional growth and development.
