About the Role

We are seeking a dedicated Care Coordinator to work with our team. As a Care Coordinator, you will be responsible for coordinating services and supports for vulnerable youth and their families.

  • Developing and implementing care plans that meet the unique needs of each family
  • Building relationships with community partners and stakeholders

Requirements

To be successful in this role, you will need:

  • Bachelor's degree in a relevant field
  • Experience working with vulnerable populations
  • Excellent communication and interpersonal skills
