Fraser Health is seeking a dedicated and compassionate Mental Health Clinician to join our team at Timber Creek in Surrey, B.C.

As a key member of our interdisciplinary team, you will provide clinical social work services to clients and their families, conducting comprehensive assessments and developing personalized care plans. About the Role

This is a full-time position that requires the successful candidate to provide clinical counselling services, conduct psychosocial assessments, and educate clients, families, and healthcare professionals on mental health issues. The ideal candidate will possess strong communication skills, the ability to work effectively in a multicultural environment, and a commitment to excellence in client care.

The estimated annual salary for this role is $85,000-$105,000 CAD, based on industry standards and market research.

  • Key Responsibilities:
  • Provide comprehensive therapeutic clinical counselling services to clients and their families, coordinating services and managing cases as part of an integrated interdisciplinary team.

  • Maintain an active role in planning for care during hospital stays and facilitating optimal discharge planning by interviewing clients and/or families to assess and determine care plans and types of services required.

  • Conduct comprehensive social work and psychosocial assessments of clients and families by interviewing them, obtaining relevant information, gathering social data regarding the client and/or family, and interpreting the data using various modalities in accordance with professional standards and clinical policies and practices.

  • Educate and provide consultation and support to clients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice in the field of mental health.

  • Maintain client records by documenting client assessments, charting client information, preparing progress notes and reports in accordance with established standards, policies, and procedures to meet regulatory requirements and document for future reference.

  • Arrange meetings to develop and coordinate care plans and community services for clients requiring facility placement and/or community support, collaborating with hospital and community services from a social work perspective to determine appropriate levels of care and the effective utilization of client resources.

  • Communicate/collaborate with mental health and related community service agencies and family physicians by making referrals to appropriate resources when follow-up is required, exchanging relevant information, and accompanying clients to pre-placement visits at residential facilities as needed.
