Job Summary

As a Personal Support Worker at VON Canada, you will play a vital role in providing companionship, personal care, and health maintenance services to clients in their homes or communities. You will work collaboratively with healthcare and personal support teams to ensure clients receive optimal levels of independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure client needs are met.
  • Contribute to the development of personalized care plans under the guidance of the Personal Support Program Manager.
  • Implement changes to care plans as needed, ensuring safety, liability, and financial considerations are taken into account.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery.
  • Perform a range of personal support tasks, including light cleaning, meal preparation, and household safety monitoring.
  • Provide personal care services, such as assistance with walking, dressing, and hygiene, as directed by the Personal Support Program Manager or immediate supervisor.
  • Offer respite care and support to clients' families, including childcare services when required.
  • Organize daily activities, prioritizing personal support and other tasks under the direction of the Personal Support Program Manager or immediate supervisor.
  • Maintain accurate records and complete daily call checks to ensure efficient service delivery.
  • Assess clients' communication patterns and abilities to ensure effective care delivery.
  • Collaborate with healthcare teams to address clients' physical, emotional, and homemaking needs.
  • Comply with Occupational Health and Safety legislation, VON policies, and procedures to ensure client safety.
  • Function in accordance with VON's risk management policies and procedures to promote client safety.
Requirements
  • High school diploma or equivalent.
  • Secondary school education preferred.
  • CCA Certification required in Nova Scotia.
  • Must have a certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  • Current Standard First Aid Certificate or CPR level C Certificate, if applicable to the work being performed.
  • Some household management experience and experience working with elderly or ill individuals or children.
  • Strong communication skills, both verbal and written.
  • Able to communicate effectively with clients and families in a clear and respectful manner.
  • Good assessment skills.
  • Awareness of community resources.
  • Ability to work effectively in a multidisciplinary team or independently as required.
  • Able to take direction well.
  • Ability to adjust to working in different social and economic environments.
  • Capacity to adhere to strict confidentiality.

Attention to detail; Lift and carry using appropriate lifting techniques; Bend, push, pull, grip, reach, kneel, crouch using good body mechanics; Walk, sit, stand, climb stairs; Hand movements.
