Key Responsibilities:

  • Conduct thorough assessments of clients' needs and develop personalized care plans to meet their unique requirements.
  • Provide personal care services, including assistance with daily living activities, mobility, and hygiene, as directed by the care plan.
  • Assist clients with household tasks, meal preparation, and shopping, as needed.
  • Monitor and maintain a safe and healthy environment for clients, reporting any concerns or changes to the supervisor or manager.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in multidisciplinary team meetings to discuss client progress and develop strategies for ongoing support.
  • Maintain accurate records and reports, as required, to ensure compliance with regulatory standards.
  • Adhere to VON's policies, procedures, and guidelines, prioritizing client safety and well-being.

Requirements:

  • High school diploma or equivalent required; secondary school education preferred.
  • CCA Certification (Nova Scotia) or equivalent required.
  • Must have a current Standard First Aid Certificate or CPR level C Certificate, if applicable to the work being performed.
  • Experience working with elderly or ill individuals or children, with a focus on household management and personal support.
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to maintain confidentiality and adhere to strict confidentiality standards.
