As a Personal Support Worker at VON Canada, you will play a vital role in providing companionship, personal care, and health maintenance services to clients in their homes or communities. You will work collaboratively with healthcare and personal support teams to ensure clients receive optimal care and support, promoting their independence and well-being.
Key Responsibilities:
- Conduct thorough assessments of clients' needs and develop personalized care plans to meet their unique requirements.
- Provide personal care services, including assistance with daily living activities, mobility, and hygiene, as directed by the care plan.
- Assist clients with household tasks, meal preparation, and shopping, as needed.
- Monitor and maintain a safe and healthy environment for clients, reporting any concerns or changes to the supervisor or manager.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in multidisciplinary team meetings to discuss client progress and develop strategies for ongoing support.
- Maintain accurate records and reports, as required, to ensure compliance with regulatory standards.
- Adhere to VON's policies, procedures, and guidelines, prioritizing client safety and well-being.
Requirements:
- High school diploma or equivalent required; secondary school education preferred.
- CCA Certification (Nova Scotia) or equivalent required.
- Must have a current Standard First Aid Certificate or CPR level C Certificate, if applicable to the work being performed.
- Experience working with elderly or ill individuals or children, with a focus on household management and personal support.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to maintain confidentiality and adhere to strict confidentiality standards.
