The successful candidate will work with the program manager to provide support to program participants during their transition from hospital to home, in accordance with the established service plan.
Key Responsibilities:
- Provide support to participants during the transition from hospital to home, in accordance with the established service plan.
- Perform basic household tasks, in accordance with the established service plan, such as preparing nutritious meals, safely handling and storing food, and ensuring the comfort and safety of the immediate environment of participants.
- Conduct a risk assessment in the participant's home using the standardized risk assessment tool.
- Foster participant and caregiver participation, including facilitating orientation to community support services and hospital discharge, in accordance with program guidelines.
- Identify and report any hazardous situations or equipment; safely handle and store hazardous materials, in accordance with the Workplace Hazardous Materials Information System (WHMIS).
- Accompany participants while maintaining a therapeutic distance.
- Report any concerns regarding participants or their environment to the program coordinator.
- Maintain confidentiality of information regarding participants, their caregivers, and the Canadian Red Cross.
Required Profile:
- Hold a minimum General First Aid and CPR Level C certification.
- Fluency in English is mandatory; knowledge of French is an asset.
- Experience working with seniors is an asset.
- Be reliable, punctual, and able to manage stress and maintain good interpersonal relationships.
- Be able to follow the care plan determined by the appropriate funding agency.
- Be autonomous and able to work in an environment with uncertainty.
- Have strong analytical and problem-solving skills; be able to work in a team in a cordial and productive manner.
Work Conditions:
- Work will primarily take place in the homes of elderly program participants.
- Working hours are 8:30 am to 4:30 pm, Monday to Friday.
- The successful candidate may be required to work on-call shifts (only during program service hours).
- Note that we do not offer relocation assistance for this position.
