Housing Stability Worker Position Overview

At London Cares Homeless Response Services, we are seeking a highly skilled and compassionate Housing Stability Worker to join our team. The successful candidate will be responsible for establishing and maintaining relationships with individuals and families experiencing homelessness, providing intensive case management services, and supporting them to achieve stability in their home and community.

Key Responsibilities

• Establish and maintain relationships with participants through the delivery of in-home intensive, ongoing, and collaborative case management services
• Support individuals and families to achieve stability in their home and community
• Help them to establish personal goals and support them to achieve those outcomes through regular communication, organizing meaningful daily activities, connecting them with other services, and creating proactive crisis support plans
• Prepare and plan for “homelessness proofing” by addressing indicators that may destabilize housing using a strengths-based approach

Requirements

• Post-secondary education in the field of social work, human services, addictions, or mental health
• 2+ years of experience working with individuals experiencing homelessness and vulnerable populations
• Complex case management experience
• Clear vulnerable police record check
• Valid Standard First Aid/CPR
• A valid “G” driver’s license

Preferred Qualifications

• Training in non-violent crisis intervention and ASSIST
• Knowledge of community resources in the City of London and system navigation
