We are seeking an experienced Licensed Practical Nurse (LPN) to join our team in Vancouver As a caregiver specialist, you will play a vital role in delivering high-quality care to our residents, ensuring their health, safety, and overall well-being.

Job Summary

The LPN will be responsible for providing comprehensive nursing care to tenants in an Assisted Living environment. This includes assessing care plans, implementing and evaluating care to meet the needs of clients, and collaborating with the Mental Health teams and other healthcare professionals to ensure seamless care coordination.

Key Responsibilities:

  • Assesses and develops individualized care plans for each client, incorporating their unique needs, preferences, and goals
  • Provides medication management, administration, and education to clients and caregivers
  • Liaises with physicians, pharmacists, and other healthcare professionals to coordinate care and services
  • Observes and reports any changes in client condition, behavior, or environment to the Manager - Supportive Living Services
  • Collaborates with the care team to develop and implement care plans that promote independence, dignity, and quality of life
  • Participates in regular team meetings, case conferences, and training sessions to stay up-to-date on best practices and industry developments

Qualifications:

  • Diploma in Practical Nursing from a recognized program
  • Current full practicing licensure with the College of Licensed Practical Nurses of BC (CLPNBC)
  • Certificates in CPR and First Aid
  • Minimum 2 years of related experience, or equivalent combination of education, training, and experience
