About the Job

We are seeking a friendly, knowledgeable, and professional Pharmacist to join our team in Truro, Nova Scotia. As a Pharmacist with Sobeys, you will be part of a dynamic team that is committed to providing exceptional patient-centered care and exceeding patients' expectations.

Key Responsibilities:

  • Provide clinical services, such as chronic health condition monitoring and management, medication reviews, health risk assessments, prescription renewals or modifications, minor ailments, or other prescribing services, within the scope of practice in your province.
  • Initiate care plans, follow up, and complete associated documentation.
  • Collect relevant patient, drug, and medical condition information.
  • Identify and resolve actual and potential drug-related problems.
  • Monitor and optimize medication adherence.
  • Provide patient counseling tailored to the needs and concerns of the patient (literacy, hearing loss, privacy, etc.) to ensure comprehensive understanding of the medication.
  • Understand all pharmacy offerings and provide each patient with a personalized interaction and intervention if appropriate.

Requirements:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy from a recognized institution.
  • Licensed Pharmacist in the province of responsibility.
  • Authorization to Administer Injections.
  • Excellent attention to detail, ability to multi-task, and work independently in a fast-paced environment.
  • Adhere to and implement all applicable company standards.
