We are currently seeking a skilled Nurse Practitioner to fill a part-time permanent position as a contracted healthcare professional responsible for delivering high-quality care to residents and clients within the Geriatric sector.

The Role
This part-time permanent position involves working as an independent contractor, responsible for delivering high-quality care to residents and clients within the Geriatric sector. As a contracted healthcare professional, you will be responsible for:
- Providing medical care delivery to geriatric clients with complex chronic co-morbidities
- Collaborating with an interdisciplinary team in the course of care delivery
- Offering high-quality customer service
- Providing clinical/medical education to clients and staff
- Participating in care conferencing
Requirements
To be successful in this role, you will need to possess the following qualifications and experience:
- A recognized Nurse Practitioner program, with Primary Health Care being preferred
- Experience in chronic disease management and geriatric care
- Valid CPR and First Aid certification
