Job Title: Care Assistant - Community Support

We are seeking a highly skilled and compassionate Care Assistant to join our team at Community Living Port Colborne-Wainfleet. As a key member of our care team, you will be responsible for providing personalized support to individuals in their homes, promoting independence, dignity, and overall well-being.

The successful candidate will have the opportunity to work with a diverse group of clients, supporting them with daily living activities, personal care, and community access. You will also contribute to maintaining a safe and healthy environment by adhering to infection control protocols and ensuring that our facilities meet the highest standards.

To be successful in this role, you will require excellent communication skills, patience, and understanding when working with individuals who may have varying levels of cognitive and physical abilities. Your ability to establish trust, empathy, and strong relationships with clients and their families will be essential.

About the Role:

This is a permanent position with 30 hours per week. The successful candidate will receive comprehensive training and ongoing support to ensure they excel in this rewarding role.

Responsibilities:
  • Provide personal care and support to individuals in their homes, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation, light housekeeping, and medication management
  • Support individuals with transportation needs, accompanying them on outings and appointments
  • Maintain accurate records of client progress, goals, and concerns
  • Collaborate with healthcare professionals and family members to develop and implement individualized care plans
Requirements:
  • CPR Certificate
  • First Aid Certificate
  • Valid driver's licence
  • 1 year of experience in a related field (personal support worker, home care assistant, etc.)
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
