Overview:

The ComForCare organization is seeking a skilled and experienced Registered Nurse to serve as the Director of Nursing for its Home Health division. This leadership position will be responsible for overseeing home health services to homebound patients in their place of residence.

Job Summary:

This role will involve supervising home health services, ensuring high-quality patient care, and promoting efficient utilization of resources to achieve desired clinical outcomes.

Responsibilities:

  • Supervise home health services, including direct clinical supervision, performance evaluations, and disciplinary action of assigned staff.
  • Evaluate and ensure that competency requirements are met for all staff members.
  • Conduct in-home supervisory visits with assigned team members to monitor caseloads, productivity, and resource utilization.
  • Develop and maintain a system of patient tracking to ensure seamless coordination of care.
  • Oversee new referrals, approval of referrals, clinician charting, visit plans, and billing productivity.
  • Prepare and submit reports on census, staffing, team productivity, and other metrics as requested by the Administrator.

Requirements:

  • A passion for serving and helping others live their best lives possible.
  • Registered Professional Nurse (RPN) licensure in the state where ComForCare operates.
  • Minimum two years of experience as an RN in community health/home health settings.
  • In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health services.
  • Strong organizational, planning, problem-solving, decision-making, and leadership skills.
