About the Opportunity

Nova Scotia Health is seeking a skilled Continuing Care Coordinator to join our team in the Central Zone.

This role will involve assessing clients' needs, developing care plans, and coordinating services to support their independence in the community.

You will work closely with interprofessional teams to facilitate hospital discharge and provide ongoing case management to ensure seamless transitions of care.

As a Continuing Care Coordinator, you will be responsible for articulating and advocating for alternative solutions to address unmet needs, while maintaining ongoing communication with clients, family members, caregivers, and healthcare teams within the client's circle of care.

The philosophy of our Continuing Care Coordinators is grounded in 'Home First': maximizing available services and resources to support clients and families in their homes and communities.

About You

To succeed in this role, you will require:

  • A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies;
  • Current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body;
  • A minimum three (3) years' experience with the professional regulatory body;
  • Preference given to candidates with more than five (5) years' experience or those who have experience working in the community;
  • Experience working in a team-based care setting is considered an asset;
  • Demonstrated knowledge and/or experience in a community or long-term care setting required;
  • Knowledge and/or experience in dedicated case management or discharge planning;
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop;
  • Knowledge of community agencies and resources;
  • Excellent assessment, interviewing, and negotiation skills;
  • Excellent verbal and written communication skills;
  • Ability to work independently and co-operatively in a busy multidisciplinary situation;
  • Ability to multi-task, prioritize workload, and manage time efficiently;
  • Valid driver's license and access to a reliable vehicle are required;
  • Competencies in other languages preferred; French an asset;
Hours and Compensation

This is a Permanent Full-Time position, 75 hours bi-weekly. The successful candidate can expect to earn $41.06 - $46.75 per hour.
