Primary Care Community Health Manager

The Timmins Academic Family Health Team (TAFHT) is seeking a skilled and experienced Clinical Lead to join our team in delivering comprehensive community health planning and promoting health initiatives. Reporting to the Clinical Director, this key role provides oversight of clinical supervision to designated Interdisciplinary Health Providers (IHPs). The successful candidate will be responsible for developing, coordinating, implementing, monitoring, and evaluating TAFHT programs and services.
Key Responsibilities- Program Development and Implementation: Develop and implement effective health promotion initiatives and strategies aimed at influencing the health behaviors of TAFHT patients and the community.
- Clinical Supervision: Provide clinical supervision to designated IHPs, ensuring they adhere to practice standards and quality guidelines.
- Quality Assurance and Improvement: Monitor patient safety and quality issues, prepare statistical reports, and communicate quality indicators and KPIs to ensure quality assurance, quality improvement, and evaluation activities for IHPs.
- Training and Education: Educate and support the privacy officer in delivering training and prevention, provide and/or facilitate training and education sessions to IHPs.
- A minimum of 2-3 years of clinical experience in a primary care, community health, or acute care setting, with a focus on working with vulnerable populations.
- Strong knowledge of chronic disease management, mental health, and addiction services.
- Proficiency in the use of computers and information technology, including electronic medical records (EMR), Practice Solutions preferred.
- Experience working in community-based settings or with underserved populations is an asset.
- Ability to speak and write in both official languages, English and French is an asset.
