Job Summary

We are seeking a skilled and compassionate Nurse to join our team as a Clinical Nurse Coordinator for our ACT team. In this role, you will provide case management services and supports to participants involved with the Flexible ACT team, coordinating medical services and medication management as needed.

Key Responsibilities
  • Assess, plan, develop, and coordinate treatment, rehabilitation, and support services for ACT program participants.
  • Provide case management services and supports to participants, including coordination of medical services and medication management.
  • Work collaboratively with the multidisciplinary team to provide wrap-around services to individuals with severe mental illness.
  • Develop and maintain relationships with community resources and services to support participants.
  • Conduct assessments and reassessments of participants to determine their eligibility for FACTT and ACT services.
Requirements
  • Minimum four years of related community experience working with individuals with serious and persistent psychiatric disabilities.
  • Nursing degree required, with a BScMH, BSc.PN, BN, RPN, or RN.
  • Registered with the College of Nurses of Manitoba and the College of Registered Psychiatric Nurses of Manitoba.
  • Knowledge of relevant legislated acts, including the Personal Health Information Act, Protection of Persons in Care Act, Mental Health Act, and Workplace Hazardous Material Information System (WHMIS).
  • Working knowledge of treatment, rehabilitation, and support in mental health, as well as experience working with individuals from diverse cultural, social, and ethnic backgrounds.
