About the Role

The Community Nurse Specialist will be responsible for delivering high-quality nursing care to clients in their homes and communities. This will include assessing client needs, developing and implementing individualized care plans, and coordinating with other healthcare professionals to ensure seamless care delivery.

Responsibilities:
  • Provide high-quality nursing care to clients in their homes and communities.
  • Assess client needs and develop individualized care plans.
  • Coordinate with other healthcare professionals to ensure seamless care delivery.
  • Conduct wound care assessments and develop treatment plans.
  • Administer IV antibiotics and perform cannulation procedures.
  • Manage CVADs and PORT-A-CATH access/de-access procedures.
Requirements:

To be successful in this role, you will need to have:

  • A minimum of 2 years' experience as a Registered Nurse (RN) or Registered Practical Nurse (RPN).
  • Current certification as a RN or RPN with the College of Nurses of Ontario (CNO).
  • Current Standard First Aid & CPR/AED (Level C) certification.
  • Mask Fit test certification.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
  • Flexibility to work weekends and on-call shifts.
