About the Role

We are seeking a highly skilled and experienced Community Health Nurse to join our Primary Care team at Flemingdon Health Centre.

As a Community Health Nurse, you will work collaboratively with an interdisciplinary team to deliver high-quality primary health care services to individuals and communities in Thorncliffe Park, Flemingdon Park, and surrounding areas.

Responsibilities
  • Provide client-centered care, assessing patients' needs, and developing personalized plans to address their health concerns.
  • Work closely with specialists, other healthcare agencies, and community organizations to ensure seamless care coordination.
  • Participate in home-bound programs, provide health teaching, and promote health education to target groups.
  • Collaborate with colleagues to identify community health needs, develop outreach programs, and support community development efforts.
Requirements
  • Registered Nurse registered in good standing with the College of Nurses of Ontario
  • Bachelor's degree in nursing
  • Minimum of three years' experience as a registered nurse within a community health centre setting, preferably in chronic disease management, assessment, and triage.
