Job Description

This is an exciting opportunity to join our team as a Registered Nurse or Registered Practical Nurse in our UHN Integrated Care Program. You will work closely with clients and families to develop personalized care plans, ensuring that each individual receives the highest level of care possible. Your expertise in community nursing will be essential in providing intricate care and specialized nursing skills within the comfort of their homes.

You will be required to assess, monitor, and make care planning/coordinating decisions, while ensuring the quality of care provided to patients by identifying and acting on factors that affect practice autonomy and delivery of quality care. You will also demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.

In addition, you will support client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement, maintain documentation standards according to the College of Nurses of Ontario, and inform nursing supervisors of any client activities and relevant concerns or changes in the client or the client care plan.

Required Skills and Qualifications
  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Membership of your Professional Association preferred (RNAO/WeRPN)
  • RN diploma/degree or RPN diploma from a recognized university or college
  • Hold a valid First Aid or CPR Certification
  • A moderate level of wound care or venipuncture knowledge and experience is an asset
  • Acute Care and Medical-Surgical nursing experience is an asset
  • Knowledge of community nursing is an asset
  • Demonstrate effective communication (written and verbal) and documentation skills
  • Demonstrate reliable attendance, punctuality, and availability for the role
