We are seeking a skilled Community Health Nurse Specialist to join our team at Closing the Gap Healthcare. As a valued member of our organization, you will have the opportunity to make a meaningful difference in the lives of our clients.

Key Responsibilities:

  • Care for client needs and provide expert wound care management
  • Report and coordinate with the HCCSS (LHIN)
  • Support school students
  • Travel between clients in the community
  • Create reports and updates on clients
  • Order supplies for patients

Requirements:

  • Minimum of two years' experience in community or clinical/hospital setting
  • Certified as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the CNO
  • Currently certified with Standard First Aid & CPR/AED (Level C)
  • Mask Fit test certification

The estimated annual salary for this position is $65,000 - $85,000 CAD, depending on qualifications and experience.
