Job Title: Primary Care Nurse Practitioner

We are seeking a highly skilled and experienced Primary Care Nurse Practitioner to join our team at Algo Medical Group Inc. in Thunder Bay, Ontario.

About the Role:

The successful candidate will be responsible for providing high-quality primary care services to patients in a community setting. This includes working collaboratively with a multi-disciplinary team, developing community-based programs, and providing care to a diverse population.

Key Responsibilities:
  • Working collaboratively with a multi-disciplinary team to provide comprehensive care to patients
  • Developing and implementing community-based programs to improve health outcomes
  • Providing care to patients with a range of health conditions, including addiction medicine
  • Documenting all encounters on an electronic medical records system
  • Collaborating with other healthcare professionals to ensure seamless care transitions
Requirements:
  • Current registration with the College of Nurses of Ontario
  • Minimum of 1 year of experience as a Nurse Practitioner
  • Primary health care experience, preferably in a community setting
  • Advanced proficiency in the use of computers and electronic medical records systems
