Nurse Practitioner (NP) - St. Joseph's Care Group

Job Summary:

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at St. Joseph's Care Group. As a Nurse Practitioner, you will work independently and collaboratively with our multidisciplinary team to provide high-quality, patient-centered care to vulnerable clients in our community.

Key Responsibilities:

  • Provide comprehensive physical and mental health assessments and treatment plans for clients
  • Work collaboratively with the GAPPS team to provide case management and coordination of care
  • Develop and maintain strong relationships with community partners and stakeholders
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures

Requirements:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult)
  • Several years of experience in community health and/or mental health
  • Membership in professional association e.g. NPAO
