Salary and Benefits

We offer a competitive salary range of $75,000 - $95,000 per year, plus benefits that include:

About the Role

As a Nurse Care Coordinator with CarePartners, you will be responsible for developing and maintaining effective care plans for our patients. This will involve working closely with patients, families, and other members of the healthcare team to ensure seamless and coordinated care. You will also have the opportunity to provide clinical services in a variety of settings, including in-home, clinic, retirement home, or school.

You will possess excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues. Requirements

To be considered for this position, you must possess:

  • A valid Registered Nurse (RN) degree or Registered Practical Nurse (RPN) diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including a two-step TB test
