About the Role

The Care Coordinator Mental Health is a key member of the interdisciplinary healthcare team, providing comprehensive and holistic care to patients across various care settings.

This role requires strong relationships between individuals and their environments, focusing on psychosocial, emotional, and spiritual well-being. Social determinants of health are central to social work theory and practice, guiding the engagement, assessment, planning, intervention, and evaluation process.

Key Responsibilities
  • Build upon patient strengths, focusing on existing coping strategies, problem-solving skills, past successes, and personal resources.
  • Develop and implement person-centered plans that address complex mental health needs.
  • Collaborate with multidisciplinary teams to provide seamless care coordination.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
Requirements
  • Bachelor's degree in Social Work from a recognized institution.
  • Current registration with the Nova Scotia College of Social Workers.
  • A minimum of two years of post-graduate clinical experience in mental health care.
  • Demonstrated knowledge of treatment theories, including harm reduction, motivational interviewing, and stages of change.
  • Strong understanding of addiction-related programming and treatment.
  • Ability to complete biopsychosocial assessments and develop individualized care plans.
  • Familiarity with local community resources and agencies, with demonstrated ability to connect patients with necessary services.
