We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in managing a caseload of diverse patients, assessing their care needs, and developing personalized care plans.
Key Responsibilities:
- Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
- Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans
- Coordinate the delivery of Home and Community Care services to patients and their caregivers
- Facilitate communication and cooperation among healthcare team members to maximize patient outcomes
- Provide regular reports to physicians, service providers, and patients as required
Requirements:
- Registered Nurse (BScN or diploma) or equivalent degree in a related field
- Related professional experience in community-based settings
- Knowledge of medical diagnoses, symptom management, and cognitive/psychosocial disorders
- Excellent communication, interpersonal, and problem-solving skills

- A competitive salary range of $40.409/hr - $43.169/hr
