Key Responsibilities:

  • Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
  • Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans
  • Coordinate the delivery of Home and Community Care services to patients and their caregivers
  • Facilitate communication and cooperation among healthcare team members to maximize patient outcomes
  • Provide regular reports to physicians, service providers, and patients as required

Requirements:

  • Registered Nurse (BScN or diploma) or equivalent degree in a related field
  • Related professional experience in community-based settings
  • Knowledge of medical diagnoses, symptom management, and cognitive/psychosocial disorders
  • Excellent communication, interpersonal, and problem-solving skills
