About the RoleThis position involves working closely with residents, families, and healthcare professionals to provide personalized support and care. As a Senior Care Coordinator, you will be responsible for coordinating resident care plans, collaborating with interdisciplinary teams, and ensuring seamless communication and service delivery.
The ideal candidate will possess excellent interpersonal and communication skills, as well as a strong understanding of gerontology and health care systems. They will also be able to work independently and collaboratively as part of a dynamic team.
Key Responsibilities- Care Planning and Coordination: Develop and implement resident care plans in collaboration with healthcare professionals and interdisciplinary teams.
- Communication and Collaboration: Foster effective communication and relationships with residents, families, and healthcare professionals to ensure seamless care coordination.
- Service Delivery: Coordinate and manage resident services, including medication management, personal care, and activities of daily living.
- Problem-Solving and Crisis Management: Respond to emergencies and critical situations requiring prompt attention and resolution.
- Documentation and Reporting: Maintain accurate records and reports on resident care and services delivered.
- Professional Development: Participate in ongoing education and training to enhance knowledge and skills in senior care and related areas.
