Job Title: Support Worker for Adults with Dual Diagnosis

Pathways to Independence is seeking a dedicated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing assisted living support for adults with dual diagnosis, mental health, and behavioral challenges.

Key Responsibilities:

  • Provide support with activities such as homemaking, personal care, community, and personal development skills in accordance with the person's Quality of Life Plan.
  • Implement established strategies and behavior management plans under the general direction of the Manager and Counsellor, DD.
  • Ensure that the person's services and supports promote their rights, choices, self-determination, independence, and builds personal and community living skills.

Requirements:

  • Secondary School Graduation, and a minimum one-year post-secondary certificate in Human or Health Sciences (Personal Support Worker or equivalent).
  • Current First Aid and CPR
  • Valid G class driver's license
  • Proof of full COVID vaccination.

Preferred Qualifications:

  • Experience working with adults with dual diagnosis, mental health, or other cognitive impairment in a residential, community, or independent living environment.
  • Ability to work independently and within a team environment.
  • Excellent written and oral communication skills.
