Job Title: Support Worker for Adults with Dual Diagnosis

Pathways to Independence is seeking a dedicated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing assisted living support for adults with dual diagnosis, mental health, and behavioral challenges.

Key Responsibilities:

  • Provide support with daily living activities, such as homemaking, personal care, and community integration.
  • Implement established strategies and behavior management plans to promote the individual's rights, choices, and independence.
  • Collaborate with the Client Services Manager and Counsellor to develop and implement individualized support plans.
  • Ensure accurate and timely documentation of client information and progress.
  • Participate in a positive team environment and contribute to a culture of excellence and respect.

Requirements:

  • Secondary School Graduation and a minimum one-year post-secondary certificate in Human or Health Sciences (Personal Support Worker or equivalent).
  • Current First Aid and CPR certification.
  • Valid G class driver's license.
  • Proof of full COVID vaccination.

Preferred Qualifications:

  • Experience working with adults with dual diagnosis, mental health, or other cognitive impairment in a residential, community, or independent living environment.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
