Job Summary

Pathways to Independence is seeking a highly skilled and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing assisted living support for adults with dual diagnosis, mental health, and behavioral challenges.

Key Responsibilities
  • Provide Support and Care: Assist individuals with daily living activities, such as homemaking, personal care, and community integration.
  • Implement Behavior Management Plans: Work with clients to develop and implement behavior management plans to promote positive behavior and independence.
  • Document Client Information: Maintain accurate and up-to-date records of client information, including progress notes and behavior management plans.
  • Collaborate with Team Members: Work collaboratively with other Support Workers, Client Services Managers, and Counsellors to ensure seamless support and care for clients.
  • Advocate for Clients: Advocate on behalf of clients to ensure their rights, choices, and self-determination are respected and promoted.
Requirements
  • Education: Secondary School Graduation and a minimum one-year post-secondary certificate in Human or Health Sciences (Personal Support Worker or equivalent).
  • Current First Aid and CPR Certification: Hold current First Aid and CPR certification.
  • Valid G Class Driver's License: Possess a valid G class driver's license.
  • Proof of Full COVID Vaccination: Provide proof of full COVID vaccination.
Preferred Qualifications
  • Experience Working with Adults with Dual Diagnosis: Have experience working with adults with dual diagnosis, mental health, or other cognitive impairment in a residential, community, or independent living environment.
  • Excellent Communication Skills: Possess excellent written and oral communication skills.
  • Ability to Work Independently and as Part of a Team: Demonstrate the ability to work independently and as part of a team.
