Job Summary

Pathways to Independence is seeking a highly skilled and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing assisted living support for adults with a Dual Diagnosis.

Key Responsibilities
  • Provide Support and Care: Assist adults with a Dual Diagnosis in their daily lives, promoting their independence and well-being.
  • Implement Care Plans: Develop and implement individualized care plans to meet the unique needs of each client.
  • Communicate Effectively: Maintain open and honest communication with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Manage Client Information: Accurately record and maintain client information, including medical history, treatment plans, and progress notes.
  • Collaborate with Team Members: Work closely with other Support Workers, healthcare professionals, and management to ensure high-quality care and services.
  • Ensure Client Safety: Identify and mitigate potential risks to client safety, adhering to established protocols and procedures.
Requirements
  • Education: Secondary School Graduation and a minimum one-year post-secondary certificate in Human or Health Sciences (Personal Support Worker or equivalent).
  • Certifications: Current First Aid and CPR certifications.
  • Driver's License: Valid G class driver's license.
  • COVID Vaccination: Proof of full COVID vaccination.
Preferred Qualifications
  • Experience: Experience working with adults with a Dual Diagnosis, mental health, or other cognitive impairment in a residential, community, or independent living environment.
  • Skills: Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Computer Literacy: Proficiency in electronic mail and other computer-based tools.
