Salary: $62,640 - $75,330 per year
Job Description:
We are seeking an experienced Nurse Practitioner to join our Complex Continuing Care program. As a key member of our interprofessional team, you will provide high-quality patient care to individuals with complex or progressive illnesses or diseases. Your clinical expertise will enable you to work autonomously and collaboratively with physicians and other healthcare professionals to develop, implement, and evaluate evidence-informed practices.
Responsibilities:
- Provide comprehensive health assessments, identify and interpret normal and abnormal findings, and diagnose health/illness conditions
- Order and interpret screening and diagnostic tests, prescribe medications, and perform procedures within your legislated scope of practice
- Counsel patients on symptom management, health maintenance, pharmacotherapy, alternative therapies, rehabilitation strategies, and other health programs
- Collaborate with the interprofessional team to improve quality of life and reduce unnecessary emergency department transfers and intensive care unit admissions
- Participate in research that contributes to improved outcomes for patients and engage in continuous professional development to maintain and further develop your competence as a Nurse Practitioner
Required Skills and Qualifications:
- Master's degree in nursing
- Current registration with the College of Nurses of Ontario (CNO) as an RN (EC) in good standing
- Demonstrated ability to provide clinical leadership and mentoring to staff, with experience in research and quality improvement methodologies
- Specialized clinical practice that supports patient populations at West Park Healthcare Centre, such as working with mechanically ventilated patients
