Job Description

We are seeking an experienced Registered Practical Nurse (RPN) or Registered Nurse (RN) to join our Palliative Care Program as a Home HealthCare Specialist. In this role, you will provide comprehensive home nursing services to patients, focusing on symptom and pain management, wound care, IV therapy, and injections.

Key Responsibilities:

  • Assess, monitor, and make care planning/care coordination
  • Evaluate care plan interventions through ongoing reassessment and evaluation results
  • Ensure the quality of care of patients by identifying and acting on factors that affect practice autonomy and delivery of quality care

Requirements:

  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Membership of your Professional Association preferred (RNAO/WeRPN)
  • RPN diploma/degree or RN degree from a recognized university or college

Preferred Qualifications:

  • Acute Care and Medical-Surgical nursing experience is an asset
  • Knowledge of community nursing is an asset
