Our Care Team Clinical Leads play a vital role in providing high-quality, client-centred care to those who need it most in their homes, schools, retirement homes, and other community settings. The Clinical Lead Home HealthCare Specialist will provide clinical support, education, and training to Personal Support Workers (PSWs) within dedicated geographical teams. This includes conducting home visits, developing client care plans, teaching PSWs evidence-based practices, and ensuring quality in-service provision.

This role requires a strong understanding of population-based risk, interprofessional collaboration, and a holistic approach to care. The successful candidate will be a Registered Practical Nurse (RPN) or Registered Nurse (RN) with CNO registration, and have a minimum of 2-3 years of experience in community health. They will also possess excellent assessment skills, superior interpersonal and communication skills, and the ability to work independently and as part of a collaborative leadership team.

Key Responsibilities:

  • Conducting home visits and developing client care plans
  • Providing clinical support and education to PSWs
  • Ensuring quality in-service provision and monitoring client safety
  • Working collaboratively with the interprofessional team to ensure client and PSW safety
  • Participating in clinical-focused committees and working groups

Required Qualifications:

  • Registration as a RPN or RN with CNO
  • Minimum 2-3 years of experience in community health
  • Excellent assessment skills and superior interpersonal and communication skills
  • Able to work independently and as part of a collaborative leadership team
  • Knowledge of population-based risk, interprofessional collaboration, and holistic approach to care

Estimated Salary Range: $55,000 - $63,000 per year
