About the Job

We are seeking an experienced Community Health Specialist to join our team. As a key member of our interprofessional team, you will work closely with clients and their families to assess their needs and develop personalized care plans. Your expertise in gerontology and chronic care will enable you to provide holistic support and education to help clients manage their conditions effectively.

Key Responsibilities:

You will conduct thorough assessments of clients' physical, emotional, and social needs, identifying areas for improvement and developing targeted care plans to address these needs.
