Key Responsibilities- Provide direct psychosocial care, counseling, system navigation, and advocacy to clients and their families.
- Collaborate with multidisciplinary teams to develop and implement care plans that promote the health and well-being of clients.
- Facilitate client self-management and provide education on relevant specialized complex care resources.
- Develop and maintain social work services to improve the health and social well-being of identified populations.
Our department provides Social Work services 7 days a week with varying hours of operation. Shift times include day and evening work, with opportunities for flexible scheduling.
