As a Care Coordinator, your responsibilities will include evaluating patient care requirements and eligibility, facilitating access to community resources, and collaborating with patients, their families, and other healthcare professionals.

Key Responsibilities:

  • Collaborate with patients and their families to assess care needs, determine service eligibility, and create personalized care and service plans.
  • Connect patients with appropriate service providers.
  • Oversee and manage the implementation of care plans.
  • Build supportive relationships with patients and their families.
  • Balance patient preferences and needs with available resources, ensuring their values are honored.
Essential Qualifications:
  • Active membership with the relevant regulatory body.
    • College of Nurses of Ontario
    • College of Physiotherapists of Ontario
    • College of Occupational Therapists of Ontario
    • Ontario College of Social Workers and Social Service Workers
    • College of Dietitians of Ontario
    • College of Audiologists and Speech Language Pathologists of Ontario
  • Degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, or Social Work.
  • Minimum of 2 years of recent experience in community health or a related area.
  • Understanding of the healthcare delivery system and community resources.
  • Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
  • Effective time management, prioritization, and organizational skills, with the capacity to work independently and collaboratively in a dynamic multidisciplinary setting.
  • Proven ability to accurately complete necessary documentation and reports.
  • A valid driver's license and access to a reliable vehicle.
  • Proficient in a Windows operating environment.
  • Compliance with our mandatory COVID-19 vaccination policy, requiring proof of vaccination status prior to employment.
Preferred Qualifications:
  • Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
  • Experience in case management or related community roles.
  • Ability to communicate in French or another second language.
