Essential Qualifications:
- Current membership with the relevant regulatory body.
- A degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, or Social Work.
- A minimum of two years of recent experience in community health or a related field.
- Strong understanding of the healthcare delivery system and community resources.
- Exceptional interpersonal, communication, assessment, and problem-solving skills.
- Proficient time management and organizational abilities, capable of working independently and collaboratively in a dynamic environment.
- Ability to accurately complete necessary documentation and reports.
- A valid driver's license and access to a reliable vehicle.
- Proficiency in a Windows operating environment.
- Compliance with mandatory COVID-19 vaccination policy.
Preferred Attributes:
- Experience working with diverse patient populations, including multicultural, homeless, palliative, and mental health groups.
- Previous case management experience or relevant community service background.
- Fluency in French or another second language is an asset.
