As a Care Coordinator, your primary responsibility will be to evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, families, and other healthcare professionals.

Key Responsibilities:

  • Collaborate with patients and their families to assess care needs, determine service eligibility, and create tailored care and service plans.
  • Connect patients with appropriate service providers.
  • Oversee and coordinate the implementation of care plans.
  • Build supportive relationships with patients and their families.
  • Balance patient preferences and needs with available resources, ensuring respect for their values and choices.
Essential Qualifications:
  • Active membership in the relevant regulatory body, such as:
    • College of Nurses of Ontario
    • College of Physiotherapists of Ontario
    • College of Occupational Therapists of Ontario
    • Ontario College of Social Workers and Social Service Workers
    • College of Dietitians of Ontario
    • College of Audiologists and Speech Language Pathologists of Ontario
  • University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, or Social Work.
  • A minimum of 2 years of recent experience in community health or a related field.
  • Familiarity with the healthcare delivery system and community resources.
  • Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
  • Effective time management, prioritization, and organizational skills, with the capacity to work independently and collaboratively in a dynamic multidisciplinary environment.
  • Proven ability to accurately complete necessary documentation, reports, and forms.
  • A valid driver's license and access to a reliable vehicle.
  • Proficiency in a Windows operating environment.
  • Compliance with our mandatory COVID-19 vaccination policy, requiring proof of vaccination status prior to employment.
Preferred Qualifications:
  • Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
  • Background in case management or relevant community experience.
  • Ability to communicate in French or another second language.
