Key Responsibilities:
- Collaborate with patients and their families to assess care requirements, determine service eligibility, and create personalized care and service plans.
- Connect patients with appropriate service providers.
- Oversee and manage the implementation of care plans.
- Establish supportive relationships with patients and their families.
- Balance patient preferences and needs with available resources, ensuring respect for their values and choices.
Essential Qualifications:
- Active membership in the relevant regulatory body.
- College of Nurses of Ontario
- College of Physiotherapists of Ontario
- College of Occupational Therapists of Ontario
- Ontario College of Social Workers and Social Service Workers
- College of Dietitians of Ontario
- College of Audiologists and Speech Language Pathologists of Ontario
- University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, or Social Work.
- Minimum of 2 years of recent experience in community health or a related sector.
- Familiarity with the healthcare delivery system and community resources.
- Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities.
- Effective time management, prioritization, and organizational skills, with the capacity to work independently and collaboratively in a dynamic multidisciplinary environment.
- Proven ability to accurately complete necessary documentation, reports, and forms.
- A valid driver's license and access to a reliable vehicle.
- Proficient in a Windows operating environment.
- Compliance with the mandatory COVID-19 vaccination policy is required.
Preferred Qualifications:
- Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
- Experience in case management or relevant community roles.
- Ability to communicate in French or another second language.
