About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Ontario Health atHome. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.

Key Responsibilities
  • Assess and Determine Patient Care Needs: Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans.
  • Link Patients with Service Providers: Connect patients with service providers, ensuring seamless access to community resources.
  • Coordinate and Monitor Care Plan Delivery: Oversee the implementation and monitoring of care plans, ensuring timely and effective delivery of services.
  • Establish a Helping Relationship: Foster strong relationships with patients and their families, providing emotional support and guidance throughout the care process.
  • Balance Patient Needs and Choices: Balance patient needs and choices with available resources, ensuring patients' values and preferences are respected.
Requirements
  • Membership in Good Standing: Hold membership in good standing with the applicable regulatory body.
  • University Degree: Possess a university degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, or a related field.
  • Experience in Community Health: Have 2+ years of recent experience in community health or a related field.
  • Knowledge of Healthcare Delivery System: Possess knowledge of the healthcare delivery system and community resources.
  • Excellent Interpersonal Skills: Demonstrate excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Effective Time Management: Possess effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment.
  • Valid Driver's Licence: Hold a valid driver's licence and have access to a reliable vehicle.
  • Proficient in Windows Environment: Be proficient in a Windows environment.
Preferred Qualifications
  • Experience Working with Diverse Patient Groups: Have experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
  • Case Management Experience: Possess case management experience or recent related community experience.
  • Language Skills: Be able to speak French or another second language.
