About the Role

As a key member of the Seniors Mental Health Outreach (SMHO) team at St. Joseph's Healthcare Hamilton, this role will collaborate with health and social support providers to enhance services for older adults with complex mental health needs.

Responsibilities
  • Develop and deliver training programs for staff in Long Term Care Homes and community support agencies in Halton Region.
  • Foster partnerships and collaborative practices to improve service delivery and resource allocation for older adults and their caregivers.
  • Analyze service gaps and integrate cross-sector solutions to meet emerging needs.
  • Support and coach staff to develop care strategies for individuals with dementia and mental illness.
About the Team

This role is responsible for serving the Region of Halton and the catchment area of the SMHO team. The main office is located at 5230 South Service Road, Burlington.

Requirements
  • Masters preferred; minimum university degree as a regulated health professional or relevant health science degree.
  • Minimum 5 years experience in geriatric psychiatry or medicine, including knowledge of multi-health sector issues.
  • Completion of curriculums such as PIECES, GPA, Dementibility, and Mental Health First Aid is a strong asset.
  • Demonstrated expertise in geriatric psychiatry/medicine screening, assessment, care planning, and intervention development.
  • Advanced facilitation, coaching, communication, and conflict resolution skills.
  • Innovative individual with enthusiasm, flexibility, and commitment to teamwork and continuous learning.
  • Working knowledge of computer programs including MS Word and PowerPoint.
  • Valid driver's license and full-time use of own car required.
Compensation Package

The estimated salary for this role is around $80,000-$100,000 per annum, depending on qualifications and experience. This figure is based on industry standards for similar positions in the region and takes into account the cost of living in Halton Region.
