LPN Clinical Lead Position Overview

Pro Vita Care Management Inc. is seeking a qualified LPN Clinical Lead to join our team in Prince George.

Key Responsibilities:

  • Provide leadership to the care department, ensuring high-quality care for residents.
  • Develop and deliver training programs, formulate individual action plans, and monitor nursing practices.
  • Maintain relationships with senior-level contacts to enhance care programs and collaborate with healthcare professionals for seamless delivery of integrated care.
  • Perform Medication Reconciliation and Admission/Discharge Audits, and participate in discharge planning activities.
  • Plan and arrange transports for upcoming appointments as required.

Requirements:

  • Graduation from a recognized Practical Nursing program.
  • Active registration with the BCCNM as an LPN.
  • Occupational First Aide Level 1 certification.
  • 2 years of Gerontological or Assisted Living experience.
  • Satisfactory proof of COVID-19 vaccinations.
  • RAI MDS Education (Gold Care or Point Click Care).
  • Effective communication skills in English.
