Job Title: Patient Care Coordinator, Mental Health

Fraser Health is seeking a highly skilled and experienced Patient Care Coordinator to join our team in White Rock, B.C.

About the Role:

The Patient Care Coordinator will provide clinical supervision, guidance, and support to nursing staff, other clinical team members, and support staff within the assigned unit(s). The successful candidate will be responsible for the day-to-day operations of the assigned unit(s), including daily patient flow, and will work collaboratively with the interprofessional care team to provide person-centered and recovery-oriented care.

Key Responsibilities:
  1. Apply established criteria and prioritize admissions/intake, lead and facilitate actions related to daily patient flow, plan for patient discharge, and appropriate patient placement/referral.
  2. Supervise nursing and nursing support staff, coordinate the work of the clinical team, and monitor the quality of care and safety of patients.
  3. Support quality patient care and excellence in practice by promoting interprofessional collaboration, team building, and staff education.
  4. Initiate/lead/support plan of care conferences and rounds, disseminate clinical information, and provide input into the development of learning objectives.
  5. Participate in or lead the investigation of complaints, follow up on complaints, and implement corrective action as appropriate.
  6. Participate in quality improvement and risk management activities, evaluate the effect of changes in practice, and generate recommendations for alternative approaches.
  7. Assist the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, and providing feedback on staff performance appraisals.
  8. Promote staff awareness and involvement in research/evaluation activities and special projects, and participate in research and special projects as appropriate.
  9. Collaborate with internal and external stakeholders, participate on designated professional or operational committees, and review, develop, implement, and evaluate evidence-informed clinical decision support tools/policies.
  10. Perform other related duties as assigned.
Qualifications:

The successful candidate will possess:

  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years of recent related clinical experience in the designated clinical area, or an equivalent combination of education, training, and experience.
  • May require current Class 5 Driver's License and access to personal vehicle for business-related purposes for applicable programs.
Skills and Abilities:

The successful candidate will possess:

  • Clinical proficiency/expertise in the designated clinical area.
  • Knowledge of community resources and health-related support services.
  • Ability to supervise and provide leadership and work direction.
  • Ability to coordinate patient assignments, in collaboration with staff.
  • Ability to monitor staff workload and patient factors and adjust patient assignments accordingly.
  • Ability to establish priorities for patient care, patient flow/discharge planning, and overall daily operations of the designated unit(s).
  • Ability to monitor the quality of patient care and ensure maintenance of standards for nursing practice and nursing support staff.
  • Ability to monitor, assess, and address performance issues based on professional standards of practice and to provide input to the Manager on formal performance reviews.
  • Ability to role model, coach, and mentor best practices in patient care.
  • Ability to identify individual or group learning priorities to meet identified program, organizational, and learner needs, in collaboration with other clinical staff.
  • Ability to support implementation of education based on changing practices, such as legislation and technology.
  • Ability to provide input into budgets and minor and capital equipment.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to work independently and in an organized and self-directed manner.
  • Ability to problem-solve and effectively deal with conflict situations.
  • Ability to operate related equipment, including applicable software, hardware, and remote/virtual technology applications.
  • Physical ability to perform the duties of the position.
