Community Access Transition Nurse

We are seeking a skilled and compassionate Community Access Transition Nurse to join our team at BC Mental Health & Substance Use Services. As a key member of our Community Transition Team, you will play a vital role in ensuring a smooth transition of care for clients leaving the Correctional Centre.
Key Responsibilities- Establish and maintain effective communication with the interdisciplinary treatment team, Correctional Centre staff, and community resources to ensure seamless transition of care.
- Conduct thorough assessments of client needs and develop personalized transition plans to address their physical and mental health requirements.
- Liaise with community resources and services to ensure clients receive the necessary support and care upon release.
- Collaborate with the inter-professional health care treatment team to validate demographic data and health information, and determine the nature and extent of needs for community health substance use support services.
- Facilitate linkages to a variety of community programs, Regional Health Authority services, community health services, substance use services, and other community support services to establish clear transition plans for the client.
- Engage in ongoing team planning, development, evaluation, and quality improvement activities to facilitate team goal setting, problem-solving, and conflict resolution.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including clients, families, and community resources.
- Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada, as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
