Job Summary

Fraser Health is seeking a Registered Nurse/Registered Psychiatric Nurse to join our team in providing high-quality community care services. As a key member of our interdisciplinary team, you will work collaboratively to assess, coach, and intervene with clients to promote their health and well-being.

Key Responsibilities
  • Establish and maintain therapeutic relationships with clients to ensure their choice and autonomy in decision-making and care planning.
  • Conduct individualized client assessments, develop care plans, and provide interventions to address complex health issues.
  • Collaborate with clients, families, and healthcare providers to coordinate care and services.
  • Develop and implement educational programs to promote health and wellness.
  • Provide health counseling and support to clients to empower them to make informed decisions about their care.
Requirements
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
  • One year recent related experience providing treatment services to individuals with complex or chronic illness, chronic pain, mental health, and/or concurrent disorders.
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes.
Skills and Abilities
  • Demonstrated knowledge, skills, and competence in areas such as individuals living with complex and/or chronic illness, chronic pain, mental illness, or substance use treatment.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
  • Knowledge of broad health care services, community resources, and their role in providing a continuum of care.
