Job Title: Community Rehabilitation Worker

Blessing Home Care and Group Home Ltd is seeking a skilled Community Rehabilitation Worker to join our team. As a Community Rehabilitation Worker, you will play a vital role in supporting individuals with various needs, including those with intellectual, learning, mental health, and physical disabilities.

Key Responsibilities:
  • Prepare comprehensive reports and case histories to document client progress and needs.
  • Conduct social research to identify effective strategies for supporting clients.
  • Implement life skills workshops and behavior management programs to promote client independence and well-being.
  • Assess client needs and develop personalized support plans.
  • Liaise with other social services agencies and healthcare providers to ensure seamless client care.
  • Accompany clients to appointments and social outings, providing emotional support and assistance as needed.
Requirements:
  • College/CEGEP education.
  • 1-2 years of experience in a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and under pressure.
  • Physical stamina to handle tasks that require standing, walking, and lifting up to 23 kg (50 lbs).
Work Environment:

As a Community Rehabilitation Worker, you will work in a group home setting, supporting clients in various locations, including urban areas. You will be required to travel to appointments and outings, using your own vehicle.
