Job Title: Community Rehabilitation Worker

Blessing Home Care and Group Home Ltd is seeking a skilled Community Rehabilitation Worker to join our team. As a Community Rehabilitation Worker, you will play a vital role in supporting individuals with various needs, including those with intellectual, learning, mental health, and physical disabilities.

Key Responsibilities:
  • Prepare and maintain accurate reports and case histories to ensure effective service delivery.
  • Conduct social research and assessments to identify clients' needs and develop personalized support plans.
  • Implement life skills workshops and behavior management programs to promote clients' independence and well-being.
  • Supervise activities and provide guidance to clients living in group homes and half-way houses.
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive support.
  • Assist clients with special needs and accompany them to appointments and social outings.
Requirements:
  • 1-2 years of experience in a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and under pressure.
  • Physical demands include lifting up to 23 kg (50 lbs).
  • Flexibility and initiative to adapt to changing situations.
Work Environment:

Our team works in a variety of settings, including group homes, urban areas, and clients' homes. We offer a dynamic and supportive work environment with opportunities for growth and development.

Language:

The primary language of work is English.

Hours of Work:

30-35 hours per week.
