Community Rehabilitation Specialist

Blessing Home Care and Group Home Ltd is seeking a skilled Community Rehabilitation Worker to join our team. As a Community Rehabilitation Worker, you will play a vital role in supporting individuals with various needs, including those with intellectual, learning, and mental health disabilities.
Key Responsibilities:- Prepare comprehensive reports and case histories to document client progress and needs.
- Conduct social research to identify effective strategies for supporting clients.
- Implement life skills workshops and behavior management programs to promote client independence and well-being.
- Assess client needs and develop personalized plans to address their unique requirements.
- Liaise with other social services agencies and healthcare providers to ensure seamless support for clients.
- Accompany clients to appointments and social outings, providing emotional support and assistance as needed.
- College/CEGEP education and 1-2 years of experience in a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse client groups.
- Ability to work in a fast-paced environment and make sound judgments in high-pressure situations.
- Physical stamina to perform tasks that may require lifting up to 23 kg (50 lbs).
As a Community Rehabilitation Worker, you will work in a group home setting, supporting clients in an urban area. You will also have the opportunity to work in various locations, including clients' homes and community settings.
