Community Rehabilitation Specialist

We are seeking a skilled Community Rehabilitation Specialist to join our team at Blessing Home Care and Group Home Ltd. As a key member of our team, you will play a vital role in supporting individuals with diverse needs and abilities.
Key Responsibilities- Case Management: Assess clients' needs and develop personalized plans to promote their independence and well-being.
- Service Coordination: Liaise with healthcare providers, social services, and other stakeholders to ensure seamless support for clients.
- Life Skills Training: Implement workshops and programs to enhance clients' life skills, including daily living, social, and vocational skills.
- Behavioral Support: Develop and implement behavior management programs to promote positive behaviors and reduce challenging behaviors.
- Client Advocacy: Advocate for clients' rights and interests, ensuring their voices are heard and respected.
- Team Collaboration: Work collaboratively with colleagues to share knowledge, best practices, and resources to achieve optimal outcomes for clients.
- Education: College or CEGEP diploma in a relevant field (e.g., social work, psychology, rehabilitation).
- Experience: 1-2 years of experience in a related field, preferably in a group home or community rehabilitation setting.
- Skills: Excellent communication, interpersonal, and problem-solving skills; ability to work in a fast-paced environment and under pressure.
- Language: Fluency in English is required.
- Work Environment: Ability to work in a variety of settings, including group homes, community centers, and clients' homes.
